The lost tomb of Sir Philip Sidney (died 1586).
In the old St.Paul's Cathedral, London.
There was a wall memorial to him.
The cathedral and wall memorial were destroyed in the Great Fire of London 1666.

The old Sidney memorial was destroyed in 1666.
This more modern memorial exists at the new St.Paul's Cathedral.
